Assessment and Enablement Worker - Learning Disability Team, North Northants Council

 

Kettering / Corby - Full Time Post, 37 hours

The North Northants Council Learning Disability Hubs are on an ambitious and exciting journey to ensure that customers locally achieve the best outcomes at the right time, and with the right support. 

 

As part of our transformation we are focussed on assessments and support planning that identify an individual’s strengths and the outcomes they want to achieve.

 

The focus of the Learning Disability Services is varied and incorporates a range of skills within the staff team.  The Assessment and Enablement role in the team will be to undertake detailed assessments to ensure that there is no delay in enabling people to move accommodation, make changes to their lives or ensuring providers are meeting identified outcomes set by, and for the customer. The assessments are in line with statutory requirements and best practice.

 

We are looking for a highly motivated, organised and creative person with experience of working within a social care background.  You will have a commitment to strengths based, person-centred values and have good communication and IT skills and enjoy working with people with a learning disability, their families and circles of support. 

 

You will be responsible for providing Care Act 2014 compliant social care assessments using the three conversation model.  You will have a passion and commitment for strengths based working, be an excellent communicator, and will have a good understanding of the particular challenges faced by adults with a learning disability and their families. You will a have a good understanding of the Mental Capacity Act 2005 and its application in community settings.
