About the Team

The Hospital Adult Social Care Team is a vibrant and fast paced team, providing social care support to all North Northants residents who need an assisted discharge from hospital.

We take referrals for North Northamptonshire residents from all hospitals, but predominantly from Kettering General Hospital (KGH) Northampton General Hospital (NGH) and the Medical Rehabilitation hospitals run by Northamptonshire Healthcare Foundation Trust (NHFT).

In line with current national guidance, we operate a Discharge To Assess (DTA) process which requires us to carry out timely assessments for those people discharged to DTA beds in care homes in the North of the County and for people discharged to their own homes with DTA packages of care or to the two Specialist Care Centres (SCC’s) in the North of the County.

The position is office based at Kettering General Hospital (although the expectation would be that you are flexible across all the sites we cover in North Northamptonshire if required).

The main duties of the role are supporting the Teams Duty function by answering the phone. As well as supporting the Team and Senior Business Support Officer to keep crucial spreadsheets and data bases accurate and up to date and other administrative tasks to help with the running of the Hospital Adult Social Care Team. The spreadsheets and databases used are crucial for both our team and the different hospitals, as they aid in communication and therefore smooth and timely discharges.

Support will be given on all aspects of the role from both team members and the Senior Business Support Officer within the Team.
