We are searching for an enthusiastic person to help us support frail vulnerable adults with their individual Reablement journeys to enable them to regain their independence. You would be working with people who have been discharged from hospital back into their own homes, across North Northamptonshire, supporting them to achieve their goals and helping them with their daily living needs.
However please be aware that North Northants Council can not employ people on a Sponsorship.
Social Care Knowledge and experience necessary. Full support and training given.
Shift Pattern: Alternating weekends and varied shift patterns.
Unit Base: 94 Saunders Close, Kettering, NN16 0AP
Our main goal is to find people who can make a difference!
--A full driving licence and access to a vehicle is essential.

What will you be doing?
Key requirements for a Needs Assessment Officer:
- You will need to be genuinely concerned about people's quality of life and be enthusiastic and able to think creatively.
- You will be enabling individuals to regain their independence.
- You would be working with the customer, setting and achieving goals in their own homes in the community.
- You may be responding to emergency situations in the community in partnership with health services and Seniors in the Team.
- You would be working with customers supporting their personal care and administering medication and nutritional needs.
- Regularly review customers' progress, monitoring the effectiveness and efficiency of the support plan and supporting Seniors within the office to make adjustments as necessary.
- Maintaining case records, including utilising department electronic record systems.
- Supporting a Supervisor to run the shift.
- Meet and greeting customers in their own homes.

You need to be physically fit and in general good health.
You need the ability to be flexible and compassionate and act with honesty and integrity and treat others with dignity and respect.
You need to be a person who takes pride in their work with a non judgemental attitude to others.
