About the role

Did we mention, no experience needed? Oh, and contracted hours so you get guaranteed pay each month, not relief contracts (unless that’s what you want of course).

We’re seeking to recruit some great new Reablement Care and Support Workers; people with the right values and positive attitude to work within the Reablement North team – could this be you?

Job Purpose:

  • Work with customers developing, implementing and updating support plans, in consultation with other interested parties, which support independence and help minimise the risk of hospitalisation or residential care.
  • Respond to customers in a crisis and provide personal care as required
  • You will be working in partnership with individuals to help regain their independence.

Pay: £11.18 per hour

Shift Pattern: 07:00-14:30 and 15:30-11:00 mixture of both or just late shifts and alternative weekends. Full time and part time contracts available

We are also offering Reablement Care and Support Worker Apprenticeships.

Our main goal is to find people who can make a difference!

- A full driving licence and access to a vehicle is essential.

- No experience is needed as full training will be provided.

What will you be doing?

Key Requirements for a Reablement Care and Support Worker:

  • We are looking to recruit individuals who are genuinely concerned about people’s quality of life and are enthusiastic and able to think creatively, supporting frail or vulnerable adults with their individual Reablement journeys.
  • We want to enable individuals to regain their independence.
  • You would be working with the customer, setting and achieving goals in their own homes in the community.
  • Respond to emergency situations in the community in partnership with health services, assessing who can be supported at home and providing short term support to maintain the person’s safety.
  • You would be working with customers supporting their personal care and administering medication and nutritional needs.
  • Regularly review customers’ progress, monitoring the effectiveness and efficiency of the support plan and making adjustments as necessary.
  • Where necessary make recommendations to senior members of the Team for ongoing care or to discontinue service provision involving informal carers and other professionals as required.
  • Maintain case records, including utilising departmental electronic record systems, in order to ensure consistency of support to customers and to assist with the smooth running of the service.
  • Maintain good links with local communities, within the company and with other agencies, to enable the customers to benefit from the most appropriate services available.
  • To provide a flexible emergency back-up service undertaking any duties arising.
  • Good communication is essential to support the Supervisors within the Reablement North team.
