About the role

An exciting opportunity has arisen in North Northamptonshire’s (NNC) Adult Social Care services for a Principal Social Worker /Principal Care Manager Transforming Care Lead.

 

What will you be doing?

As the Adult Social Care (ASC) Transforming Care Lead you will be supporting NNC/ASC to discharge their statutory duties under the Integrated Care Board (ICB) Transforming Care Policy.

You will be liaising with our partners in health, other local authorities, and partnership agencies as part of this key role.

You will be responsible for organising attendance or attending CETR/CTR meetings and to provide information for the Transforming Care Steering group which feeds into monthly meetings with NHS England.

You will be instrumental in developing positive professional relationships within all areas of this role, and to participate in regular Dynamic Support Register meetings, plus any other meetings required for this role.

Part of this role will be to offer advice and guidance in respect to the Transforming Care process, Dynamic Support Register etc and to develop and deliver training and development sessions for ASC teams.

You will be using your line management skills to support identified colleagues as their manager and use your knowledge of complex situations to advise and guide as required for ASC staff.

 

 

 

About you

You will have a sound knowledge of the Integrated Care Board Transforming Care Policy and other relevant legislation, which is to include knowledge of Mental Health legislation, with a focus on section 117.   

 

You will have the passion and commitment to drive forward the transforming care agenda, and the ability to work collaboratively with all stakeholders to achieve this.

You will be an excellent team player, confident, assertive, be able to manage conflict effectively, with an ability to think flexibly and creatively in a solution focused and person-centred way.
