About the role

Are you passionate about supporting people with care and support needs living in North Northamptonshire to progress and live independently? 

As part of our transformation, we are focussed on assessments and support planning that identify an individual’s strengths and the outcomes they want to achieve.

The Assessment and Enablement role in the team will be to undertake detailed assessments to enable people to move accommodation, make changes to their lives or ensure providers are meeting identified outcomes set by, and for the individual. The assessments are in line with statutory requirements and best practice. 

Job title: Assessment and Enablement Worker (AEW)

Job status: 37 hours, full time position

Salary:  £33,366 - £35,235

What will you be doing?

You will be responsible for providing Care Act 2014 compliant social care assessments using the three conversations model. You will have a passion and commitment for strengths-based working, be an excellent communicator, and will have a good understanding of the particular challenges faced by adults with social care needs and their families. You will a have a good understanding of the Mental Capacity Act 2005 and its application in community settings.

About you

Applicants must hold a valid Driving Licence and have access to a car.

The three conversation model paving a new way of working in Adult Social Care and we are looking for workers who are enthusiastic and passionate to work with us on this journey.

We are looking for a highly motivated, organised and creative person with experience of working within a social care background.  You will have a commitment to strengths based, person-centred working; have good communication and IT skills and have skills to work with people with a range of social care needs.
