About the role

For this role you will need to:
Assist with the provision of positive intervention and support for adults with mental health needs; physical and / or learning difficulties.
Promote independence and community living and help minimise the risk of hospitalisation or admission to residential care
North Northamptonshire Council Adult Social Care provides a Three Conversation approach, a model that re-emphasises focus on people, their strengths and the networks through the development of community hubs.
We are looking for Community Support Workers who believe it is important to ensure people have choice and control over their own support needs. We want people who are committed, like us, to focus on the person, their strengths, progression and networks rather than routinely assessing people for services and eligibility. The three conversation model paving a new way of working in adult social care and we want Community Support Workers who are enthusiastic and passionate to work with us on this journey.
Post holders may work with any combination, or all, of the following care groups: older people, people with physical, sensory, cognitive impairments, people with a learning difficulty and people with mental health needs.
Basic Requirements:
Educated to GCSE or equivalent
NVQ/QCF Level 2 in Social Care or equivalent
Proven experience of health or care work
Good verbal and written communication skills sufficient to write clear, concise and accurate reports and to enable effective communication with a number of different agencies.
