About the role

This is an exciting opportunity to join us in the new brokerage function – The Community Resource (Brokerage) Team – Fixed Term 1 Year contract.

What will you be doing?

Provide operational services within the Community Resource team for all NNC provider packages, to include community and acute for all adults over 18 years.

Provide quality assurance to social care requests using a strength based model.

Provide an effective, person focused support service to internal and external colleagues ,to be responsive and a pro-active communicator.

You will be a motivated individual that is passionate about find the right support for the individual we broker services for.
